Benefit Eligibility

You are eligible to participate in UnityPoint Health benefits the first day of the month following your hire date or becoming newly benefit eligible. You will be eligible for benefit coverage if you are a:

  • Full-Time Benefit Eligible – A team member who is regularly scheduled or budgeted to work between 64 and 80 hours per pay period. (0.8 – 1.0 FTE)
  • Part-Time Benefit Eligible – A team member who is regularly scheduled or budgeted to work between 32 and 63 hours per pay period. (0.4 – 0.79 FTE)
  • Weekend Package Work Type – A weekend package (WEP) team member who is regularly scheduled or budgeted to work between 64 and 80 hours per pay period will be eligible for full-time benefits. A WEP team member who is regularly scheduled or budgeted to work between 32 and 63 hours per pay period will be eligible for part-time benefits.

*Note: Dubuque Union Team members should refer to their collective bargaining agreement.

Health Insurance

UnityPoint Health offers two medical plan options for you to choose from: Network Plan and Health Savings Plan. You may also choose to waive medical coverage. Both plans include prescription drug coverage and pay 100% of preventive services when care is received at a network provider. Your share of the cost of the medical services you receive will differ. The Network Plan will have a higher biweekly payroll cost for premiums than the Health Savings Plan while the cost of care under the Health Savings Plan will be higher than the Network Plan. If you enroll in the Health Savings Plan, you will be able to participate in a tax-advantaged health savings account that allows you to use those account funds to pay for current or future healthcare expenses. Both medical plan options offer a network of doctors, hospitals and health care specialists who deliver quality care according to network standards and have agreed to lower, preferred rates for covered services. Precedence Inc.

Mental health and substance use disorder benefits are available through the plan. Precedence is the administrator of these benefits and claims are paid by HealthPartners.

Virtual Care Program

UnityPoint Health Virtual Care is a convenient, on-demand option for care that is available 24/7 through secure video on your smartphone, computer or tablet. You can get a diagnosis, recommended treatment and prescribed medications sent to local pharmacies, all within minutes.

UnityPoint Health Virtual Care features:

  • Convenient Care – Virtual Urgent Care eliminates the need to travel and brings the care to you. You don’t need an appointment and you do not have to be a current patient of UnityPoint Health. Simply log in or create a MyUnityPoint account and use the video camera on your mobile phone, tablet or computer for a face-to-face consultation.
  • 24/7 Access –We understand that illnesses and injuries don’t always happen during regular business hours. That’s why Virtual Urgent Care gives you online access to a team of UnityPoint Health medical experts 24/7, including holidays and weekends. It’s care that’s built around your schedule!
  • Trusted Medical Experts –Virtual Urgent Care is staffed by our team of board-certified UnityPoint Health providers. This expansive network gives you the same excellent quality of care you expect from UnityPoint Health without leaving the comfort of your own home. Please be aware that ourVirtual Care providers are only licensed to see patients in Iowa and Illinois and cannot treat patients who are currently outside of those states.

Dental Insurance

UnityPoint Health offers two dental plan options:

  • Basic Plan
  • Premier Plan

You may also choose to waive dental coverage. The Premier Plan has a higher biweekly payroll cost for premiums than the Basic Plan. While both dental options provide coverage for preventive and diagnostic services, the Basic Plan does not cover major restorative services or orthodontia.

Delta Dental is our claim administrator and they contract with dentists and other dental care providers across our service areas. When you visit a dental provider in the PPO network, your cost of care will be less than going to a non-PPO dental provider. To see if your dentist participates in the Delta Dental PPO network, you can go to deltadentalia.com or call (800) 544-0718 to speak with a Delta Dental representative.

Vision Insurance

UnityPoint Health offers vision coverage through Avesis. Under the plan, you may purchase your eyeglasses and contacts at the eye care provider of your choice. However, when you use an Avesis network provider, you receive the highest level of plan benefits and have the lowest out-of-pocket costs.

Flex Spending Accounts

Flexible Spending Accounts (FSAs) reduce your taxable income by allowing you to set aside pre-tax dollars toward eligible health care and dependent daycare expenses throughout the year. This pre-tax deduction lowers your taxable income, reducing the amount of tax you owe on your bi-weekly paycheck.

  • Health Care FSA – This account is for IRS eligible health care expenses for you and your dependents. Eligible expenses include medical, dental, orthodontia, vision and hearing aid expenses not covered by another health benefit plan.
  • Limited Use FSA – If you enroll in the Health Savings Medical Plan, you cannot have a regular Health Care FSA, but you can have a Limited Use FSA. With the Limited Use FSA, IRS eligible expenses include dental, orthodontia and vision expenses – not medical or prescription drugs.
  • Dependent Care FSA – This account can be used to pay dependent care expenses for children under 13 or adult dependents that can’t care for themselves. IRS qualified expenses enable you and your spouse to go to work and include in-home child care, licensed day care, preschool facilities, before- or after-school programs, and elder care.

Life Insurance

Basic Life Insurance
UnityPoint Health provides group term life insurance and Accidental Death and Dismemberment (AD&D) insurance automatically at no cost to eligible team members. Eligible team members have a coverage amount of one times their base annual pay, up to $100,000.

Voluntary Life Insurance
Benefit eligible team members are also offered the ability to purchase additional voluntary life insurance and Accidental Death and Dismemberment (AD&D) insurance at favorable group rates for the team member, their spouse or domestic partner and children (up to age 26).

  • Employee coverage can be elected in increments of $10,000 up to the lesser of eight times the annual salary or $500,000.
  • Spouse or domestic partner coverage can be elected in increments of $10,000 up to $100,000. Amounts that exceed $50,000 will require EOI.
  • Children coverage, up to age 26, can be elected as a flat amount of $5,000 or $10,000 per child

Basic and Voluntary life insurance include accidental death and dismemberment (AD&D) coverage, and team members will not have to purchase it separately. AD&D coverage pays a monetary benefit if the covered individual passes away as a result of an accident. It will also pay all or part of the coverage amount if the covered individual suffers a dismemberment injury or other covered loss due to an unfortunate accident.

Disability Insurance

UnityPoint Health provides both short term and long term income protection for team members in the event they become disabled and cannot work due to a non work-related illness or accidental injury.

  • Short-Term Disability (STD) – This benefit is paid for entirely by UnityPoint Health at no cost to our team members. Coverage begins on the first of the month following your hire date or date you change to a benefit eligible status. The STD benefit will pay after seven days of continuous disability and for most team members will replace 60% of your regular weekly earnings, to a maximum of $2,500 per week, for up to 26 weeks.
  • Long-Term Disability (LTD) – This benefit is paid for entirely by UnityPoint Health at no cost to our benefit eligible team members. If you remain totally disabled and unable to work for more than 180 days, you may be eligible for LTD benefits, which replace up to 50% of your monthly base pay, up to a maximum of $5,000 per month for part-time team members and $10,000 per month for full-time team members. Monthly LTD benefits will be reduced by Social Security and any other disability income you are eligible to receive, such as Workers’ Compensation.
  • Long Term Disability Buy-Up – Eligible team members can purchase an additional 10% of LTD coverage that will increase their total monthly benefit to 60% of their monthly pay.

Whether you need assistance writing your will, disputing a traffic ticket, or protecting yourself against identity theft, Legal Insurance and Identity Theft Protection from ARAG is here to help. For only $9.46 per pay period you and your covered dependents will have access to a nationwide network of attorneys who will work with you to address and resolve life’s legal, financial, and identity theft issues, such as:

  • Consumer and fraud protection
  • Wills and estate planning
  • Real estate
  • Family law
  • Civil damage claims (defense)
  • Criminal matters
  • Government benefits
  • Small claims court
  • Tax issues
  • Traffic matters
  • Debt-related matters
  • Landlord disputes

This service can track your credit activity or online identity and you are notified immediately of suspicious activity. If your identity is stolen, the ARAG Identity Theft Protection program provides full-service restoration including access to Certified Identity Theft Restoration Specialists and reimbursem*nt for up to $1 million for expenses associated with restoring your identity.

ARAG is here to meet the needs of all team members including, but not limited to, those with a disability, a veteran, or a member of the LGBTQ+ community.

Hospital Indemnity

We know that medical expenses can pile up quickly, especially when you, your spouse or domestic partner, or child are admitted to a hospital or rehabilitation facility. To help alleviate some of that financial pressure, UnityPoint Health offers benefit eligible team members the opportunity to enroll in a Hospital Indemnity Plan that pays a benefit for admission and each day spent in the hospital or rehabilitation facility. That is money that can be used for anything it is needed for, whether for hospital bills or other costs that come up while recovering. See your Enrollment Guide for eligibility and details.

Adoption Assistance

UnityPoint Health will grant up to $6,000 in reimbursem*nt per child to benefit eligible team members for expenses related to the process of the legal adoption of a child. If both parents work for UnityPoint Health, the total amount of reimbursem*nt per legal adoption of a child is up to $6,000 per child. For reimbursem*nt, a benefit eligible team members must complete an application and follow the proper plan steps.

Critical Illness

Critical Illness Insurance pays a lump sum benefit if you are diagnosed with a covered illness or condition. You can use the benefit payment for any purpose you choose. You can purchase coverage for yourself, spouse/domestic partner under age 70, and your children under age 26. If you leave UnityPoint Health, you can take this coverage with you. The following coverage amounts are available:

  • Employee – $10,000 | $20,000 | $30,000
  • Spouse/Domestic Partner – $10,000
  • Children – $5,000 per child

The plan also pays a wellness benefit of $50 once a year when you, your covered spouse/domestic partner or your eligible children complete a wellness screening. Wellness benefits for your children are limited to a maximum of $100 per year.

Accident Insurance

Accident Insurance pays you benefits for specific injuries and events resulting from a covered accident. You can purchase coverage for yourself, spouse/domestic partner under age 70, and your children under age 26. If you leave UnityPoint Health, you can take this coverage with you.

The amount paid depends on the type of injury and care received. For those who experience an accident claim and visit a UnityPoint Health facility, you will receive an additional 25% benefit up to $1,000. You can use the payments for any purpose you choose.

Pet Insurance

Like health insurance for you and your family, pet insurance is coverage for dogs and cats that can help you be prepared for unexpected vet costs.

Each pet’s premium will be unique based on age, breed, location, as well as what coverage amount you select. Coverage is flexible and customizable, so you can choose the plan that works for you. Options include:

  • Levels of coverage from $500 – unlimited
  • $0 – $2,500 deductible options
  • Reimbursem*nt percentages from 50% – 100%

Retirement & Financial Planning

Your financial health is very important to us. That is why UnityPoint Health invests in your future by offering a Retirement Savings Plan, or 401(k), where we automatically provide a core contribution equal to 2% of pay into each team member’s account. In addition to the core contribution, UnityPoint Health also provides a 50% matching contribution for each dollar team members contribute to the plan, up to the first 6%. (3% maximum match). This means you could receive up to a 5% contribution from UnityPoint Health!

  • You can join at any time once you become eligible. Your contribution will be automatically set at 2%. You can change this amount at any time through the Fidelity website.
  • You can make contributions to your plan account through convenient paycheck deductions and can place them in various investment options offered by the plan. Contributions can begin as early as within 30 days of your start date. Health Savings Account (HSA)

To help create an easier and more personal way to plan for retirement with an ability to view your account balances for HSA and 401k in one place, UnityPoint Health partners with Fidelity to administer our Health Savings Accounts.

If you enroll in the Health Savings Medical Plan, you are eligible to contribute to a Health Savings Account (HSA). An HSA is a personal savings account you can use to pay for qualified out-of-pocket health care expenses with pretax dollars. Your HSA can be used for the expenses of you, your spouse and applicable dependents, even if they are not covered under the same medical plan as you.

Contributions to an HSA account are not taxed, and once your HSA balance reaches $2,000, you can invest it in a variety of options. Your account (including any investment earnings) grows tax-free, and if the funds are used to pay for qualified health care expenses, they are also spent tax-free.

If you enroll in the Health Savings Medical Plan option, UnityPoint Health will contribute to help offset your out-of-pocket costs, regardless of your own contributions. In 2024, per the IRS, UnityPoint Health team members are eligible to contribute up to $4,150 when enrolled in individual health care coverage and $8,300 when enrolled in family health care coverage. These amounts include UnityPoint Health contributions.
